Authorization to communicate with insurer

By checking the box below and signing this form, I authorize DB Restoration, LLC DBA Dry Bros to communicate with my insurance company, its adjusters, representatives, and other claim-related parties regarding the property damage claim identified above.

This authorization permits Dry Bros to provide and discuss restoration-related information, including damage documentation, photographs, estimates, scopes of work, invoices, scheduling, and progress updates. It does not authorize Dry Bros to settle my claim, endorse checks, make coverage decisions, or act as my legal representative.

I understand that I remain responsible for my insurance policy, claim decisions, deductibles, coverage obligations, and any amounts not covered by insurance. I may revoke this authorization in writing at any time; revocation will not affect communications already made in reliance on this authorization.
